Other than the Applicant Information Box, This form shall be  filled out entirely by the neighboring parties to a proposal.  The information provided will be considered by the Board in its evaluation of the proposal.  The neighbor(s) filling out this form shall hold legal title to the property they are  representing.  If the property owner has assigned an agent to respond on  his, her, or their behalf, please indicate so by filling in the Agent's Name and Title in the entry provided.  Form questions are typically provided with “yes” or “no” answer options with further explanation requested depending upon the specific answer.   However, all questions may be provided with expanded responses – whether the answer is affirmative or negative.   Provide additional sheets as suitable to your response.

[1] Story poles are required to provide a reasonable depiction of the proposed project.  However, the plans are the official documentation of the proposal.  If approved, the project must be built in accordance with the plans, not the story poles.
